Health Requirements

The Illinois School Code and New Athens C.U. School Dist. #60 Policy require that students adhere to the following physical/immunization and dental schedule:

Physical & Proper Immunization:

    • Pre-K

    • Kindergarten

    • 6th Grade

    • 9th Grade

    • Or the first year your child attends.

Dental:

  • Kindergarten

  • 2nd Grade

  • 6th Grade


Diphtheria. Tetanus, Pertussis (DPT):

  • Has received 4 or more doses of DPT or TD, with 2 doses received no less than 4 weeks apart and the final dose being a booster. The booster must have be received no less than 6 months after the last dose and on or after the 4 th birthday. All students in sixth thru 12th grades, who have not already received a dose Tdap, will need to receive one dose of Tdap regardless of the interval since last DT, Td, or DtaP.

Polio (OPV):

  • Has received 3 or more doses of OPV with 2 doses received no less than 6 weeks apart and the final or ending dose being a booster. The booster must have been received no less than 6 months after the last does and on or after the 4 th birthday. If series is given in any combination 4 or more doses are required. New Regulation for Kindergarten starting 2016-2017 school year. Has received 4 or more doses of the same type of OPV/IPV with 3 doses received no less than 4 weeks apart and the final or ending dose being a booster. the booster must have been received no less than 6 months after the last dose and on or after the 4th birthday. If series is given in any combination, 4 or doses are required.

Measles:

  • Has received 2 doses of measles vaccine.

Rubella:

  • Has received 2 doses of rubella vaccine.

Mumps:

  • Has received 2 doses of mumps vaccine.

Note: The vaccine for the above listed Measles, Rubella, and Mumps is combined as MMR Vaccine. Two (2) MMR’s are required.

Hepatitis B:

* The hepatitis B vaccination is a requirements for children entering 6th through 12th grades. A series of three vaccinations is required. The first and second dose must be space at least 4 weeks apart. In the current rues, the interval between the second dose and third dose must be at least two month. However, it is medically recommended that the interval between the first and the third dose should be at least four months to ensure optimal protection. Longer intervals between the first and third dose are also acceptable, at the physician’s discretion.

Chickenpox or Varicella Vaccine:

*Any child entering Kindergarten through 2nd and 6th through 11th grades, must show proof of having received 2 doses of varicella.

Meningococcal (MCV4)

*Any child entering 6th thru 7th grades will be required to show proof of receiving one dose of meningococcal conjugate vaccine (MCV4) given on or after the 10th birthday. 12th graders must show proof of receiving 2 doses with the second dose being received after 16 years of age. If first dose is received after 16 years of age than only 1 doses of MCV4 is required.

You may get any needed immunizations from your family physician or from the Health Department. If you wish to get these immunizations from the Health Department please call immediately for an appointment since limited space is available. Students will be excluded from school if not adequately immunized.

Dental Requirements

As of July 1, 2005, a new law was passed in the state of Illinois. This law states that all children in kindergarten, 2nd grade and 6th grade must present proof of having been examined by a dentist. Your child must present a completed dental examination form to the school.

In addition to the physical, dental, and immunizations required for school entry, the State of Illinois also required Lead Poisoning Screening prior to kindergarten entry and Diabetes Screening for all kindergarten, 6th, and 9th grade physicals. This should be done by your family physician at the time of the physical exam. These screenings are now mandatory and your child will be excluded until completed.

Eye requirements

All kindergarten and new students to the state of Illinois are required by Illinois law, to have an eye examination by a optometrist or ophthalmologist

Students will be excluded from school if they are not adequately immunized.
